HEALTH HOME SERVICES (HHS)
Oasis Health HHS provides hope and support through personalized home care visits that work towards reaching each client’s behavioral health goals, addressing somatic health and other predetermined health targets. Clients are assigned a care provider who helps assess and prioritize physical health needs and then creates a comprehensive care plan. Each client’s care giver also coordinates with other health care providers as needed; promoting a coordinated and comprehensive health care approach. Specifically, HHS offers:

- Weight Maintenance
- Diabetes Control
- Accessing Benefits
- Transitional Care Needs
- Comprehensive Care Management
- Care Coordination
- Health Promotion

- Social Support Referral
Additional Information
In order to participate in Health Homes, clients must be currently enrolled in either our Psychiatric Rehabilitation Program or Mobile Treatment Services Program. A care giver/provider will be assigned to each client and all services through this program are performed at the client’s home.
